During our 15 minute conversation we can see how I can help you, schedule our first appointment, and verify your insurance benefits, or discuss other payment options.

To be well prepared for our first appointment, please fill out the intake form and make additional notes for yourself such as symptoms or questions that you wish to address during our first session.

If you have an extensive medical history, please bring or send me a summary containing the dates of injury as well as previous and/or current treatments. This will reduce the time we will spend creating an understanding of where you started, how you have progressed and/or why your recovery has not progressed past your current limits.

We will also clarify accessibility issues: There are a few stairs and manual doors coming into the building. If there is any reason we feel there will be any difficulty getting into the building, I will assist you.


To start off smoothly, for our first appointment, please send the following documents prior to our first appointment via Email. You will get my Email address during our first conversation:

  • Photos of insurance card, front and back

  • Copy of your Photo ID, front

  • Any information required by Medicare or other insurance, such as a prescription for physical therapy evaluation and treatment.

  • Prescription for physical therapy evaluation and treatment from your PCP, doctor or dentist – required after ten visits or thirty days per New York State law
    Please note: Your plan may require the prescription to be submitted to the insurance before treatment.

  • Images: any relevant Xrays, MRIs or CTScans (particularly treating scoliosis and/or joint injuries)

  • List of medications

Step 2

Get your Individual Plan

During our first appointment, we will focus on your diagnosis in its entirety: I will assess your movement, strength and coordination. I will listen closely to your needs and fears so we both understand your goals for recovery.  In order to get to the root causes of your pain and dysfunction, I will explore all relevant aspects of the musculoskeletal system and how your nerves respond to movement and posture. We will discuss your medical history, previous treatments and current medication.

We will then discuss my recommendations for a personalised plan to reduce pain, increase strength/stamina and restore movement. Depending on your plan, we will schedule a series of visits and get you started on your healing journey.

Based on anatomy, physiology, treatment techniques and multiple disciplines, we start each session assessing your current condition.

Hands-on-treatment combined with consistent specific exercise will foster positive change, while you learn to maintain maximal independent self-care. We will adjust your customized home program as needed during your recovery.


Regular monitoring and feedback will contribute to an improved overall sense off well-being while improving your physical condition and quality of life.

Frequently Asked Questions

How soon can I get an appointment?

My lead time for appointments is mostly one to two weeks.

Do I have to see a doctor before I see you?

In NYS patients have direct access to physical therapy without the need to see a Physician first. Be aware that some insurances may require that you see a doctor prior to seeing your PT. After ten treatments or thirty days after the first treatment, you should follow-up with your doctor, If you need ongoing care.

Is physical therapy covered by insurance?

Physical therapy is covered by insurance differently based on how the specific plan covers therapy. We should find out those details and discuss how your benefits cover treatment out of network.

Do you accept Medicaid or Medicare?

I am a provider for the standard policy of Medicare. I am not in-network with the managed care plans. I am not a provider for Medicaid.

Why should I see an out-of-network therapist?

My treatment approach includes individual Manual therapy and training programs during one-on-one sessions to provide the type of care I would want to receive and feel is valuable. Reach out to me to discuss your options. I will give you a recommendation about how to continue your search if I am not the right fit for you.

How long will the treatment take? How often do I need to come?

I specialize in customized care, so each treatment plan is different based on your goals, lifestyle and budget. I do not plug-and-play with templates for every person. To work on an acute symptom, I recommend treatment two-three times a week to start. Typically symptoms are changing by 4 visits, if they don’t change immediately. Ongoing care is determined by what you need and how our intervention is helping.

What are your office hours?

My regular office hours are from 9 a.m. to 5 p.m. You can book a time slot via my online scheduler. I am flexible with time and location.

How much does it cost?

Pricing differs based on location and length of care.

Is Physical Therapy painful?

Pain typically occurs due to dysfunction. In order to recover, the injured area – muscles, tendons, associated connective tissue – needs to be moved in order to restore or develop a more optimal pattern of movement. Sometimes this controlled motion might cause some discomfort, and personalized treatment for each patient aims to minimize that while achieving our treatment goals.

How long will treatment take until I am well?

If treatment is not obviously showing any improvement within a reasonable time frame (4 visits over 2 weeks) we should discuss this directly and change to a new approach if indicated or stay the course due to a long-term treatment plan.

Will exercises make me sore?

If you have never done certain exercises before, slight soreness may occur, typically one or two days later. However, it will feel better with movement and will resolve over a week. We will talk about how to manage being active and control comfortable movements without being limited by or worsening any symptoms.

What should I wear for a physical therapy session?

Please wear clothing that is comfortable to move in and, depending on the injury, allows the injured area to be exposed for treatment.

Are you available for telehealth treatment?

Yes, I am available for support by phone or video conference to provide guided self-care or advice.

What is your cancellation policy?

Please remember to cancel an appointment you are unable to keep 48 hours prior to the appointment.
